We are conveniently located in the Slabtown district in NW Portland. Our office is inside the Northwest Center for Orthopedics and Rehabilitation Building at 1515 NW 18th Avenue – Suite 400, Portland, OR 97209.

PARKING

Free parking is available under our building at street level, as well as in the parking lot diagonal of us on the northeast corner of NW 18th Ave and Raleigh St. Street parking in the neighborhood is free.
